You will be offered customized recommendations tailored to your needs, wishes, and goals at the forefront of every interaction. Functional life skills Occupational Therapy assessments and standardized cognitive assessment tools are used to create and implement personalized action plans, which include consulations, coaching, guidance, education, caregiver training and support. A new sense of hope, resilience, and connection can return as you are skillfully guided through the process of aging with more ease, peace, and confidence.

Our Occupational Therapist combines her clinical observation skills with valid & reliable standardized assessment tools to identify and implement practical solution oriented recommendations, education, guidance, and support that is tailored to your specific circumstances and needs. You can choose to work with her as much or little as you need as no insurance company dictates how your time together is spent. Our Occupational Therapist’s expert knowledge of how cognition impacts safety and function with Activities of Daily Living (basic daily tasks), Instrumental Activities of Daily Living (complex cognitively demanding tasks required to live independently) and Life Skills in general makes her a unique member of your care team. You will be offered solutions to set you up with conditions that allow thriving to the absolute best of your ability.

You will be supported, educated, and included as you team with a qualified coach. We understand how the entire family unit is impacted by challenges in navigating the eldercare system, not just the elder themselves. Our offerings are holistic, client centered, family unit focused, and tailored to suit your individual needs. Our services are carried out with care, compassion, and mindfulness.

No. Thrive Elder Care Coaching & Consulting is an independent consulting practice and all associated fees are an out of pocket expense.

The rate of frequency will depend on the complexity of your situation as well as how much or little you would like to partner with us. On average, sessions are scheduled once a week for the first 4-8 weeks. The frequency may drop to once a month or once every 3-6 months after the bulk of the assessments, action plan implementation, and education have been completed. Occasional check-in sessions are recommended as circumstances change and minor adjustments may be required.

Yes. There are great therapeutic benefits to working in your own home environment. Assessments are more accurate in situations that are not just simulated, environmental modifications are easier to recommend and implement, and hands-on caregiver training can be completed with return demonstrations assuring good carryover of new learning for maximum safety and independence. Remote options are available upon request although sessions in your own home setting is highly recommended.

It is a person-centered approach to elder & dementia care that focuses on utilizing existing cognition and retained skills to promote independence, dignity, and continued participation in meaningful activity rather than fixating on cognitive decline. Core principles include modifying the environment creating safe, structured, and visually accessible cues to encourage safe exploration of tasks and increased independence with wayfinding; simplifying tasks by breaking them down into manageable, step-by-step activities to promote success and build confidence; giving individuals purposeful responsibilities to provide a sense of belonging and community; utilizing multi-sensory experiences to evoke memories and restore a sense of calm; and allowing individuals to make personal choices and treating them with the utmost dignity.
